Step #1: Purchase a USAV Tryout Membership

Membership Notes:
• Athletes attending summer tryouts (Girls 14U–18U and all Boys age groups) may use their current 2025–26 season membership for tryouts.
• Once an athlete accepts a spot on a team, they must upgrade their tryout membership to a full-season membership.
• You may choose to purchase the full-season membership now, but USAV does not issue refunds if the athlete does not join a club team this season.

Step #2: Complete the USAV Medical Waiver Form

• CLICK STEP 2 BUTTON to download and fill out the USAV Medical Waiver Form

 

 

• Be sure the form is fully completed and signed.

Step #3: Upload Medical Waiver During Pantheon Registration

• During the Pantheon tryout registration process, you will be prompted to upload the completed USAV Medical Waiver Form.
• Be sure to have the form ready to upload before beginning registration.

Step #4 Payments 

Payments are $25 with cash, check, or Venmo. Online Payments are $29. To pay online find your team, and use the YELLOW PAY NOW BUTTON.
